B104.3 Birthday Club

1. Eligibility:

The B104.3 FM Birthday Club is open to individuals who are at least 18 years old at the time of entry.

2. How to Enter:

Participants must complete the Birthday Club entry form available on the B104fm.com website.

All required fields must be accurately and completely filled out for the entry to be valid.

3. Entry Period:

Entries must be submitted by noon the previous business day before the big day to get them on the list and to participate in contest. 

Entries submitted after deadline, will not be considered.

Entry information will stay on file for future Birthday Club contests.

4. Prize:

One person will be selected as winner every Friday from that week's birthday club members.

The winner will recieve a certificate for one (1) free Dairy Queen Birthday Cake from Dairy Queen in Farmington, Mo. or Potosi, Mo.

Prizes are non-transferable and cannot be exchanged for cash.

5. Notification of Winners:

Winners will be notified via a phone call from Odle Media Group staff from the contact information provided on the entry form.

If a winner cannot be contacted within 30 days of the initial notification, the prize may be forfeited, and an alternate winner may be selected.

6. Publicity:

By entering the Birthday Club, participants grant B104.3 FM and B104fm.com the right to use their name, likeness, and entry for promotional purposes without additional compensation.

7. Privacy:

Information provided on the entry form will be used only for the purpose of administering the Birthday Club and in accordance with the privacy policy of B104.3 FM and B104fm.com.

8. General Conditions:

B104.3 FM and B104fm.com reserve the right to disqualify any participant who violates these terms and conditions.

B104.3 FM and B104fm.com reserve the right to modify, suspend, or terminate the Birthday Club in the event that it becomes corrupted or technically impaired.

9. Governing Law:

All regular contest rules apply.

By participating in the B104.3 FM and B104fm.com Birthday Club, participants agree to abide by these terms and conditions.
